Insurance Accepted

HomeAid.com accepts various medical insurances such as Medicare, Champus, Blue Cross, Blue Shield, United Healthcare, Foundation Health Systems, HealthNet, Seniority Plus and others.

If you are supplementing payment with your insurance, print-out and complete the Health Insurance Claim Form. Please read the instructions if you need assistance (if you are unable to read Adobe Acrobat files, download Adobe Acrobat Reader first). Mail your completed claim form (don't forget to have your doctor sign and to sign the form yourself!) along with a copy of your insurance member card, prescription, co-payment, and order. Do not place your order online as that will result in your credit card being charged the full amount. We will process your request and contact you should we have any questions.
